Permission Form

To SES Caregivers,

At Shutesbury Elementary School, we use Google Workspace for Education and third party educational apps. We are seeking your permission to provide and manage accounts for your child. Google Workspace for Education is a set of education productivity tools including Drive, Docs, Slides, Classroom, Gmail, Calendar, and more. At Shutesbury Elementary, only students in grades 3-6 use Google Workspace for Education accounts to complete assignments, communicate with their teachers, sign into their Chromebooks and learn 21st century digital citizenship skills. All students in grades K-6 use educational apps and websites for learning and practicing skills such as phonics, reading, writing, math, communication, digital skills and more.

The notice posted on our website at: Shutesburyschool.org - Services - Technology - Google Workspace  Notice to Caregivers provides answers to common questions about what Google can and can’t do. In addition, it also provides a detailed list of the “Core Services and Additional Services” students may access. Please read it carefully, let us know if you have any questions, and then sign below to indicate that you’ve read the notice and give your consent. 

Additionally, Shutesbury Elementary School is committed to providing K-6 students with access to educational web-­based tools that promote academic excellence. In order to create accounts for our students, we must comply with federal regulations that require us to obtain caregiver consent. To view the list of third party apps used with students please visit our website at: Shutesburyschool.org - Services - Technology - Websites and Apps. This page will be updated as new apps are added to support educational goals.

I give permission for Shutesbury Elementary to create/maintain a Google Workspace for Education account for my child in grades 3-6 only and for Google to collect, use, and disclose information about my child only for the purposes described in the posted notice.

I give permission for Shutesbury Elementary to create/maintain accounts for my child in grades K-6 to access third party educational apps as listed on the school website.
